GP Central – Privacy Policy

1. Purpose

We collect, use, and store personal and health information to provide safe, effective, and coordinated healthcare to you and your whānau.

2. What information we collect

We may collect the following information:

• your name, date of birth, address, and contact details
• medical history, clinical notes, diagnoses, and treatment plans
• medications, allergies, immunisations, and test results
• referral and specialist information
• appointment, billing, and administrative information

3. How we collect your information

From you

We collect information directly from you:

• during consultations
• through enrolment forms
• via phone, email, or patient portal (MyIndici)

4. Information we receive from other organisations

By accepting healthcare services from GP Central, you agree that we may collect information about you from other agencies and organisations for the purpose of maintaining an up-to-date health record and providing safe, coordinated, and high-quality primary healthcare.

This may include:

From other health providers and shared health records

• laboratory, radiology, and other test results
• authorised national or regional health information systems (where available)
• hospitals, emergency departments, after-hours providers, specialist services, allied health providers, and community care providers, including discharge summaries and specialist letters
• information received when we follow up referrals or seek advice from hospitals or specialists
• screening programmes (e.g. bowel, breast, cervical), including results and attendance or non-attendance notifications

From non-health agencies and organisations

Where relevant to your care, or where permitted or required by law:

• ACC
• Police
• legal representatives
• Oranga Tamariki
• insurance companies
• Ministry of Social Development (Work and Income)
• employers (e.g. fitness to work)
• New Zealand Police regarding firearms licensing

In most cases, you would reasonably expect this information to be shared with your general practice as part of your care. Due to the volume and nature of information received, it is not always practicable for us to notify you each time information is received.

5. Why we collect your information

We use your information to:

• provide medical care and treatment
• coordinate care with other providers
• maintain accurate health records
• manage appointments, billing, and administration
• meet legal and regulatory obligations

6. Who we may share your information with

We may share your information, where appropriate, with:

• healthcare providers involved in your care
• laboratories, radiology services, and pharmacies
• Health New Zealand / Te Whatu Ora and PHO providers
• ACC and other government agencies (where required)
• trusted service providers supporting our systems (e.g. patient management systems such as Indici)

We only share information as permitted by law and for purposes related to your care.

7. Complaints, concerns, and legal matters

If concerns are raised about the care or services we provide, we may disclose relevant health information to regulators, insurers, indemnity providers, or legal advisers for the purpose of managing and responding to those concerns.

8. Storage and security

Your information is stored securely in electronic and/or paper records.
We take reasonable steps to protect your information from loss, unauthorised access, or misuse.

9. Your rights

Under the Health Information Privacy Code 2020, you have the right to:

• access your personal and health information
• request correction of your information

Requests can be made by contacting us using the details below. We will respond in accordance with the Privacy Act.

10. Retention of information

We retain health information in accordance with the Health (Retention of Health Information) Regulations 1996.
